We have 2 documents which are essentially questionnaires (attached as Word and PDF here) and we would like to create the questions into a survey via Google Forms. We would like to have the questions created as they are in the documents here, but we would then like to add 3 options that people can choose to appear to the right hand side of each answer as "green", "orange" and "red". People filling in the questionnaire at this stage would only be “marking” each questions and answer option as green/orange/red at this stage.
The 2 documents are similar in content and overlap in some areas but have slightly differing questions and essentially need combining/merging so we have all questions between the two onto the Google Forms questionnaire - but without duplicates. We have gone through the second document (the PDF) and struck out in red lines the ones we can see are not needed but this was a quick review and if you see any that are not struck through but are actually a duplicate please do omit.
The purpose is to have a questionnaire template that is not filled in at this stage but that we are going to mark each question as "red", or "orange" or "green" - to the right hand side of the questionnaire.
People giving answers should only be able to choose one of red/orange/green, but the “rule” in Google Forms needs to be that one of the 3 colours is chosen for each question.
Below are the 2 documents that need putting into Google Forms and the Excel is included an example of how things should read in terms of titles, etc. Hopefully this helps to give the context on how the final document would read. It can be completely ignored if this confuses matters!
I think it would be best to approach the project as follows:
1. Work through the Word document to format correctly as it’s currently just a converted PDF
2. Go through the PDF to remove the areas struck out and then lift the extra content into the Word document to combine
3. Review the combined document and ensure the different sections are in the right places (they are in slightly different orders on each document)
4. Create the Google Forms survey as per the format above – but just one area of the survey and send to me to preview and check format before continuing to complete the full task.
This is a big job to type up as I don't think there is a shortcut to setting this up, so please only apply if you have the time to prioritise this in the next 2 days. I would want the final GoogleForm ideally by the end of the week, or at the latest, mid next week please.
I am open to cost on this so long as it is done timely and to the standard laid out above.
